Your Personal Data will be processed in Singapore and Hong Kong and will be accessed by a UBS team specifically set up to process your enquiry. Your Personal Data will be collected, used and processed by UBS AG to process your enquiry about your entitlement to a margin spread adjustment (the "Purpose"). In particular, we will reconcile your Personal Data against our internal database of impacted clients and reach out to you to follow up on your request or with the outcome of our assessment.

Please note that your Personal Data will be subject to applicable laws abroad (including data protection, banking secrecy and client confidentiality laws), which may allow your Personal Data to be accessed by local authorities or other third parties.

We will retain your Personal Data for as long as necessary to fulfill the purpose for which they were collected or to comply with legal, regulatory or internal policy requirements.

You confirm that all the Personal Data submitted with this form is true, accurate, complete, not misleading and provided on a voluntary basis. Failure to do so may impact the assessment of your entitlement to a margin spread adjustment.
