Complete a personal wellness challenge

You can complete one challenge from each of the two categories for a total of two challenges:

  • Weight tracking
  • Sleep

Each participant 1, 2 can earn:

  • $50 per participant for each challenge

Complete a movement challenge

Complete a movement challenge each month in partnership with the American Heart Association.

Daily activity tracking for $0.50 per day (up to $182.50 for 365 days)

Complete the hydration challenge

Replace sugary drinks with water for at least 5 days a week for 4 consecutive weeks or drink 8, 8 oz glasses each day for at least 5 days a week for 4 consecutive weeks.

$50 per participant

Complete Blood Pressure Readings with Hello Heart

Complete 2 blood pressure readings per month for 3 consecutive months

Complete an annual preventative exam*

Complete an annual preventative exam*.
Have one of the following exams or screenings to meet this requirement:

  • Adult Males: Annual Physical, Annual Dental Exam and Colon Cancer Screening*
  • Adult Females: Annual Physical, Annual Dental Exam, Well-Woman Preventative Care Visit, Cervical Cancer Screening (Pap Smear), Mammogram (Diagnostic and Routine)* and Colon Cancer Screening*

*Please note that minimum age and other requirements may apply to certain medical services. Call your carrier for more information.

Participate in one or more organized walking or running event

Complete an organized official walking or running event with a distance equal to or greater than 5K, or other equivalent race or longer such as a Triathlon or Tough Mudder.

Participate in Weight Watchers

Enroll in Weight Watchers and complete five check-ins from 01/01/26 to 12/31/26

Current Weight Watchers members don't need to re-enroll; but will need to complete five check-ins during the incentive period to earn their HSA credit.

By completing a quick mental health check-in, you can gain a better understanding of the importance of your overall well-being. Complete 6 mental health check-ins to receive the wellness incentive.

Complete a journaling exercise

Keep a journal at least three times a week for six weeks in a row. Journaling is a great way to reduce stress, boost health, and improve well-being. You can write for 15 minutes or record voice or video files. Your thoughts and feelings are your own. So your journals will stay with you.

You can receive this incentive for select webinar events sponsored by UBS Financial Wellness at one session per month. The monthly UBS Financial Wellness Spotlight webinars are not eligible towards HSA incentives.

Complete an Assessment with a UBS Financial Wellness Coach4

UBS Financial Wellness provides you with access to complimentary, confidential one-on-one consultations with licensed financial coaches. Coaches can help you at all stages of your financial journey with various financial questions. During your initial conversation, a coach will work with you to complete an assessment to help you identify your goals and see where you stand today. Only one conversation with a coach per year is eligible for the HSA incentive. Schedule a consultation here.

**An annual maximum of $400 per participant, regardless of number of wellness activities completed 1, 2

1. You are only eligible to earn up to the maximum wellness contribution (based on your coverage level), no matter how many wellness activities you participate in. You can earn up to $400 for employee only coverage, and $800 if you cover any family member.

2. Employees and their spouses/domestic partners may earn UBS contributions to the employee’s HSA by completing wellness activities via the MyActiveHealth portal; dependent children cannot earn incentives. If you cover yourself and a child or children, but not a spouse/domestic partner, you will have access to complete more wellness activities via MyActiveHealth in order to earn the maximum $800 incentive. Dependent children over the age of 18 can, however, register and use MyActiveHealth. They just don't receive the incentive.

4. UBS Financial Wellness Activities are only available for UBS employees.

IMPORTANT NOTE: In any health or biometric assessment and any other similar assessments as part of this, UBS is not requesting or receiving any family medical history or genetic information as part of the wellness incentive. Further, when it comes to healthcare, your privacy is protected by federal law. Your personal information and results are yours and yours alone, although you may wish to share them with your doctor. Our plans will report overall claims information and wellness participation results to UBS (no individual data) so we can track the health risks that challenge our population as a whole, as well as the results of our wellness programs. This gives us an opportunity to develop and modify our programs and resources so they are most beneficial to our employees.

UBS will not see your or your family’s individual information, health status or the details of your participation any Wellness programs. With respect to Wellness Programs, UBS will only be notified by ActiveHealth of the completion of a given activity.
Having a Health Savings Account (HSA) -- or if applicable, a Health Reimbursement Account (HRA) -- through UBS provides you an opportunity to earn or receive contributions from the firm. But if you don't enroll in UBS's Core or Core Plus plan, you cannot open an HSA through UBS. Under IRS rules, you cannot contribute to a HSA unless you are covered under a high deductible health plan or consumer directed health plan (CDHP).

UBS contributions to your Health Savings Account are contingent upon your being employed by UBS on the date the contributions are made to the account. UBS reserves the right to change or terminate its benefit plans at any time, including, without limitation, changing the amount of any UBS contributions to HSAs.
