The Forms Kiosk is a convenient tool for UBS Connect users to complete and download forms online or in PDF format. This tool is available 24/7 allowing users to generate documents quickly and easily.
It offers three types of documents based on the user's delegation agreement:
How to get access
To access the Forms Kiosk, users need to sign the Delegation contractual documents and navigate to the "Documents" tab on the UBS Connect homepage. The Forms Kiosk offers two types of forms: PDF forms, which can be filled out in Adobe Acrobat Reader, and online forms, which can be completed directly in the Forms Kiosk.
Once the form is completed and signed, it must be returned to UBS via postal mail.
