Have you heard about the Forms Kiosk?

The Forms Kiosk is a convenient tool for UBS Connect users to complete and download forms online or in PDF format. This tool is available 24/7 allowing users to generate documents quickly and easily.

It offers three types of documents based on the user's delegation agreement:

  • Delegation Agreement (Identification and Access): Users can download various forms, including Automatic Exchange of Information (AEI), Confirmation of tax disclosure, Correspondence Instructions, and other forms helpful for the FIM and end-client.
  • Delegation Agreement (Identification): Users can access essential "Know your client" documents, such as Power of attorney for the management of assets for FIM, Authorization for the use of email by the Financial Intermediary, and Forms K, A, S, & T.
  • No Delegation: Users can still access FIM documents, such as Power of attorney for the management of assets for FIM and Authorization for the use of email by the Financial Intermediary.

How to get access

To access the Forms Kiosk, users need to sign the Delegation contractual documents and navigate to the "Documents" tab on the UBS Connect homepage. The Forms Kiosk offers two types of forms: PDF forms, which can be filled out in Adobe Acrobat Reader, and online forms, which can be completed directly in the Forms Kiosk.

Once the form is completed and signed, it must be returned to UBS via postal mail.
