All you have to do to enter the competition is to submit this form with your name and address. Participants should be aware that their name and other information may be viewed by third parties when it's been sent via internet. Therefore confidentiality cannot be guaranteed. You may only submit one form. The competition is open to everyone. Participation is free and does not involve any obligation. The closing date for the competition is 31. July 2008. The winner will be notified by post only. The winner will be drawn from the correct answers. The prize cannot be paid out in cash. No correspondence will be entered into regarding the draw.
