How to apply
1. Print out the form «UBS KeyClub Registration
».
2. Give us all of your UBS client numbers (accounts) so that you can collect all the points you are
due.
3. If you hold several UBS credit cards, please state all your card numbers.
4. Please send the completed and signed form to:
UBS AG
KeyClub Service
P.O. Box
4002 Basel.
5. If you do not yet have a UBS VISA Card/UBS MasterCard, but would like to collect more bonus
points when you spend, we would be happy to send you the relevant application forms.
Good to know
All UBS clients with a named account and all holders of UBS credit cards can join UBS KeyClub. The scheme cannot be extended to numbered accounts, legal entities, clubs, associations or company cards.
