We’re here with personal advice if you need it.
Please contact our Key Account Management on +41-44- 828 33 07. If you already have a UBS VISA Corporate Card, you will find all of the forms you need to apply for additional cards below. Should you have questions, please do not hesitate to contact your Key Account Manager.

Complete the form in full and sign it. Please enclose original excerpt from the Commercial Register. NB: This form only needs to be completed with your initial card order. If your company already has a UBS VISA Corporate Main Account, you only need to complete the UBS VISA Corporate Card order form.

You can choose between CHF, EUR and USD for each individual cardholder listed on the card application. The request must have a legally valid signature (signature of the cardholder and legally valid signature of the company – please note individual/collective signature).

The list of authorized signatories must be completed by domiciliary companies and other legal entities whose corporate documents do not provide information on their authorized signatories. There must be given the signatures of all organs, agents and third parties authorized to sign with respect to UBS AG.
